Patient safety, including medication safety, should be the primary area of focus for all hospitals.  Despite the call to action from the Institute of Medicine in To Err is Human published in 2000, many hospitals have made far too little progress to ensure patients are safe.  This has necessarily resulted in an increased focus on safety from accrediting bodies and regulatory agencies, and many hospitals are having difficulty trying to keep up.
